The headlines read “Pharmacy counterstaffs clear drug shortage”. To find a reliable remedy, the Medicines and Healthcare products Regulatory Agency (MHRA) had to rely on clinical trials, and this was the first step in doing so. During a recent visit to the UK’s General Medical Council (GMC), a team of scientists examined the potential for tadalafil (Cialis) to reduce cardiovascular risks in people taking the drug, and found that patients who had taken the drug had significantly fewer heart attacks, strokes, and other hospitalisations, compared with those who had not. They also noted that patients who were taking the drug had a lower risk of experiencing any of the common side effects mentioned above, such as erectile dysfunction and back pain, which was not mentioned in the trials.

“A significant number of patients have had at least one adverse event reported in the clinical trials of this drug. The drug should therefore be considered safe for daily use,” said Dr Sarah Webb, chairwoman of the Medicines and Healthcare products Agency (MHRA) from West Midlands. “Tadalafil has been a reliable and widely used treatment for erectile dysfunction for more than 60 years and can still be prescribed as an alternative in some circumstances.”

In the UK, trials were carried out for Cialis (tadalafil) over the course of three years. In February 2019, the MHRA approved the first clinical trial for tadalafil, to compare the drug to sildenafil (Viagra). The trial included men aged 18 to 64 years old, with a mean age of 62.2 years. All men taking the drug had an average heart attack within the previous four weeks. Of the trial’s 27 participants, 8 had a single episode of angina.

The trial was designed to be used by people with erectile dysfunction, but it was not designed to be taken by patients taking other drugs. The trial involved just one person and involved 8 participants. The trial was funded by Pfizer (now Pfizer), a British multinational pharmaceutical company with manufacturing facilities in the UK.

In the UK, men taking tadalafil (for erectile dysfunction) have an average cardiovascular risk of 2.1 per 1,000 person-years, compared with 2.2 in the general population, according to data from the National Health Service (NHS). There was also an increase in the number of cardiovascular deaths in those taking tadalafil (between 20% and 30%) compared with those on sildenafil (between 15% and 20%).

This study also examined tadalafil’s potential impact on cardiovascular risks among men with erectile dysfunction, looking at the association between taking the drug and heart attacks, strokes and hospital admissions in patients with cardiovascular disease.

The cardiovascular effects of tadalafil were studied across a range of doses, with a mean daily dose of 20 mg for tadalafil 10 mg, which was equivalent to the average daily dose of sildenafil 20 mg, and 10 mg for sildenafil, which was equivalent to the average daily dose of tadalafil 20 mg. The mean heart attack rate was significantly reduced in those taking tadalafil 10 mg, compared with those taking the drug at the same daily dose, but the number of events was not different. There were no differences in hospitalisations, deaths, or in rates of cardiovascular death, heart attack or stroke between the two groups.

Dr Webb also noted that “the findings of our clinical trial are important, and we are working to make sure this drug is safe for daily use.”

However, the MHRA had not yet issued an update on the drug’s safety for patients taking tadalafil. The MHRA will still take into account other aspects of the drug, including whether patients were taking other medications, how long they were taking the drug, and how many doses were needed. The drugs may change in nature or frequency, so further clinical trials will be carried out to make sure the effects of tadalafil continue to be well-tolerated.
